RESUMO

The purpose of this study is to establish and apply a correction method for titanium alloy implant in spinal IMRT plan, a corrected CT-density table was revised from normal CT-density table to include the density of titanium alloy implant. Dose distribution after and before correction were calculated and compared to evaluate the dose deviation. Plans were also copied to a spinal cancer simulation phantom. A titanium alloy fixation system for spine was implanted in this phantom. Plans were recalculated and compared with the measurement result. The result of this study shows that the max dose of spinal cord showed significant difference after correction, and the deviation between calculation results and measurement results was reduced after correction. The method for expanding the range CT-density table, which means that the density of titanium alloy was included, can reduce the error in calculation.

OBJECTIVE@#To study the feasibility and potential benefits of beam angle optimization (BAO) to automated planning in liver cancer.@*METHODS@#An approach of beam angle sampling is proposed to implement BAO along with the module Auto-planning in treatment planning system (TPS) Pinnacle. An in-house developed plan quality metric (PQM) is taken as the preferred evaluating method during the sampling. The process is driven automatically by in-house made Pinnacle scripts both in sampling and scoring. In addition, dosimetry analysis and physician's opinion are also performed as the supplementary and compared with the result of PQM.@*RESULTS@#It is revealed by the numerical analysis of PQM scores that only 15% patients whose superior trials evaluated by PQM are also the initial trials. Gantry optimization can bring benefit to plan quality along with auto-planning in liver cancer. Similar results are provided by both dose comparison and physician's opinion.@*CONCLUSIONS@#It is possible to introduce a full automated approach of beam angle optimization to automated planning process. The advantages of this procedure can be observed both in numerical analysis and physician's opinion.

Advanced radiotherapy technology enables the dose to more accurately conform to the tumor target area of the patient, providing accurate treatment for the patient, but the gradient of the patient's radiation dose at the tumor edge is getting larger, which putting forward higher requirements for radiotherapy dose verification. The dose verification system software KylinRay-Dose4D can verify the patient's pre-treatment plan and the in vivo/on-line dose during the patient's treatment, providing important reference for the physicist to modify the radiotherapy plan and ensuring that the patient receives accurate treatment. This study introduces the overall design and key technologies of KylinRay-Dose4D, and tests the pre-treatment plan dose checking calculation and 2D/3D dose verification through clinical cases. The test results showed that the 2D/3D gamma pass rate (3 mm/3%) of KylinRay-Dose4D reconstructed dose compared with TPS plan dose and measured dose is larger than 95%, which indicating that the reconstructed dose of KylinRay-Dose4D meets the requirement of clinical application.

To ensure a reliable verification of a radiation detector, the right parameters for this response verification must be determined and a specific characterization on the detectors of interest must be performed. These were the main pillars of this study, where four Geiger-Müller at the University of Costa Rica's Cyclotron Facilities' main laboratories were studied and characterized using a 137Cs source. First, a verification of the inverse-square law was performed to corroborate the correct measurement by the detectors as the distance from a 137Cs source to the detectors was varied using a new design for a positioner support to ensure repeatability. This verification yielded a potential fit curve with and equation D=670635 x-1.961 (error percentage of 1.95%) and an R2 value of 0.9836. Then, using combinations of copper plates of widths 1.0 mm and 2.0 mm as attenuators between the source and the detectors, the mass attenuation coefficient for copper was obtained only as a reference value for future calibrations of the detectors. The result for this value was 0.040 cm2 /g. The results obtained in this study and the method developed to achieve these results will serve as a base for calibrations of the detectors at these facilities, which will ensure the safety of the patients and personnel in this building.


Para asegurar respuesta correcta de un detector de radiación, se deben determinar los parámetros correctos para esta verificación y debe realizarse una caracterización específica de los detectores de interés. Estos fueron los pilares principales de este estudio, donde se estudiaron y caracterizaron 4 detectores Geiger-Müller en los laboratorios principales del Ciclotrón de la Universidad de Costa Rica utilizando una fuente radiactiva de 137Cs. Primero, se realizó una verificación de la ley del inverso-cuadrado para corroborar la medición correcta de los detectores según se varía la distancia entre la fuente de 137Cs al detector utilizando un diseño nuevo de un soporte posicionador para la fuente que asegura la repetibilidad entre experimentos. Esta verificación resultó en una curva de ajuste potencial de ecuación D=670635x-1,961 (porcentaje de error de 1,95%) y un valor de R2 de 0,9836. Luego, utilizando combinaciones de placas de cobre de espesores 1,0 mm y 2,0 mm como atenuadores entre la fuente y los detectores, se obtuvo el coeficiente de atenuación másico para el cobre como un valor de referencia para futuras calibraciones de los detectores. Este resultado fue de 0,040 cm2/g. Los resultados obtenidos en esta investigación y el método desarrollado para lograr estos resultados servirán como una base para una futura confirmación metrológica calibraciones de los detectores en estos laboratorios, lo cual colaborará con la seguridad y protección radiológica de pacientes y trabajadores en este edificio.

El objetivo de esta investigación fue determinar la relación entre la dosis de radiación recibida y la práctica en intervención médica. Se estudiaron estadísticamente los reportes dosimétricos de profesionales que han laborado en intervencionismo con un Arco en C en el Hospital José Carrasco Arteaga de la ciudad de Cuenca, Ecuador. Estos informes se basan en lecturas bimensuales de los dosímetros TLD utilizados por el personal para seguridad radiológica. En total, se analizaron 124 personas que trabajaron en el período de noviembre de 2013 a marzo de 2018, es decir, 24 informes. La dosis promedio anual para las enfermeras de quirófanos es de 1,32 mSv, 1,59 mSv para el personal de Urología, 1,74 mSv para el personal de Gastroenterología, 1,78 mSv para el personal de Hemodinamia y 2,33 mSv para el personal de Traumatología. Se concluyó que la dosis promedio para cada práctica no excede la dosis de restricción de 1/3 de la dosis permitida para trabajadores ocupacionalmente expuestos. Intervencionismo en Traumatología resulta ser la práctica con mayor dosis recibida y la más baja perteneció al personal de Enfermería de Quirófanos. Estos resultados son de gran utilidad para saber qué nivel va acorde con cada práctica médica.


The objective of this research was to determine the relationship between the radiation dose received and the practice in medical intervention. The dosimetric reports of professionals who have worked in intervention with a C-Arm at the José Carrasco Arteaga Hospital in the city of Cuenca, Ecuador, were studied statistically. These reports are based on bi-monthly readings of the TLD dosimeters used by the staff for radiological safety. In total, 124 professionals who worked in the period from November 2013 to March 2018 were analyzed, that is, 24 reports. The average annual dose for operating room nurses is 1.32 mSv, 1.59 mSv for urology staff, 1.74 mSv for gastroenterology staff, 1.78 mSv for hemodynamic staff and 2.33 mSv for trauma staff. It is concluded that the average dose for each practice does not exceed the restriction dose of 1/3 of the dose allowed for occupationally exposed workers. Traumatology is the practice with the highest dose received and the lowest belongs to the operating room nursing staff. These results are very useful to know which level is consistent with each medical practice.

The efficacy and toxicity of radionuclide therapy are believed to be directly related to the radiation doses received by target tissues; however, nuclear medicine therapy continues to be based primarily on the administration of empirical activities to patients and less frequently on the use of internal dosimetry for individual therapeutic planning. This review aimed to critically describe the techniques and clinical evidence of dosimetry as a tool for therapeutic planning and the main limitations to its implementation in clinical practice. The present article is a nonsystematic review of voxel-based dosimetry. Clinical evidence pointing to a correlation between the radiation dose and therapeutic response in various diseases, such as thyroid carcinoma, neuroendocrine tumors and prostate cancer, is reviewed. Its limitations include technical aspects related to image acquisition and processing and the lack of randomized clinical trials demonstrating the impact of dosimetry on patient therapy. A more widespread use of dosimetry in therapeutic planning involves the development of user-friendly dosimetric protocols and confirmation that dose estimation implies good efficacy and low treatment-related toxicity.

ABSTRACT The aim of this study was to create and test a new mice 3D-voxel phantom named DM_BRA for mice and human first-estimation radiopharmaceutical dosimetry. Previously, the article reviews the state-of-art in animal model development. Images from Digimouse CT database were used in the segmentation and on the generation of the voxelized phantom. Simulations for validation of the DM_BRA model was performed at 0.015, 0.1, 0.5, 1 and 4 MeV photons with heart-source. Specific Absorbed Fractions (SAF) data were compared with literature data. The organ masses of DM_BRA correlated well with existing models based on the same dataset; however, few small organ masses hold significant variations. The SAF data in most simulated cases were statistically equal to a significant level of 0.01 to the reference data.

This study presents the characterization of an X-ray irradiator through dosimetric tests, which confirms the actual dose rate that small animals and cells will be exposed to during radiobiological experiments. We evaluated the linearity, consistency, repeatability, and dose distribution in the positions in which the animals or cells are placed during irradiation. In addition, we evaluated the performance of the X-ray tube (voltage and tube operating current), the radiometric survey (leakage radiation) and safety devices. The irradiator default setting was established as 160 kV and 25 mA. Tests showed that the dose rate was linear overtime (R2=1) and remained stable for long (constant) and short (repeatability) intervals between readings. The mean dose rate inside the animal cages was 1.27±0.06 Gy/min with a uniform beam of 95.40% (above the minimum threshold guaranteed by the manufacturer). The mean dose rate inside the cell plates was 0.92±0.19 Gy/min. The dose rate dependence with tube voltage and current presented a quadratic and linear relationship, respectively. There was no observed mechanical failure during evaluation of the irradiator safety devices and the radiometric survey obtained a maximum ambient equivalent dose rate of 0.26 mSv/h, which exempts it from the radiological protection requirements of the International Atomic Energy Agency. The irradiator characterization enables us to perform radiobiological experiments, and assists or even replaces traditional therapy equipment (e.g., linear accelerators) for cells and small animal irradiation, especially in early research stages.

PURPOSE: This study aimed to evaluate whether the deformable image registration (DIR) method is clinically applicable to the safe delivery of re-irradiation in hepatocellular carcinoma (HCC). MATERIALS AND METHODS: Between August 2010 and March 2012, 12 eligible HCC patients received re-irradiation using helical tomotherapy. The median total prescribed radiation doses at first irradiation and re-irradiation were 50 Gy (range, 36-60 Gy) and 50 Gy (range, 36-58.42 Gy), respectively. Most re-irradiation therapies (11 of 12) were administered to previously irradiated or marginal areas. Dose summation results were reproduced using DIR by rigid and deformable registration methods, and doses of organs-at-risk (OARs) were evaluated. Treatment outcomes were also assessed. RESULTS: Thirty-six dose summation indices were obtained for three OARs (bowel, duodenum, and stomach doses in each patient). There was no statistical difference between the two different types of DIR methods (rigid and deformable) in terms of calculated summation operatorD (0.1 cc, 1 cc, 2 cc, and max) in each OAR. The median total mean remaining liver doses (M(RLD)) in rigid- and deformable-type registration were not statistically different for all cohorts (p=0.248), although a large difference in M(RLD) was observed when there was a significant difference in spatial liver volume change between radiation intervals. One duodenal ulcer perforation developed 20 months after re-irradiation. CONCLUSION: Although current dose summation algorithms and uncertainties do not warrant accurate dosimetric results, OARs-based DIR dose summation can be usefully utilized in the re-irradiation of HCC. Appropriate cohort selection, watchful interpretation, and selective use of DIR methods are crucial to enhance the radio-therapeutic ratio.

According to the International Atomic Energy Agency (IAEA), a relatively significant number of radiological accidents have occurred in recent years mainly because of the practices referred to as potentially high-risk activities, such as radiotherapy, large irradiators and industrial radiography, especially in gammagraphy assays. In some instances, severe injuries have occurred in exposed persons due to high radiation doses. In industrial radiography, 80 cases involving a total of 120 radiation workers, 110 members of the public including 12 deaths have been recorded up to 2014. Radiological accidents in industrial practices in Brazil have mainly resulted in development of cutaneous radiation syndrome (CRS) in hands and fingers. Brazilian data include 5 serious cases related to industrial gammagraphy, affecting 7 radiation workers and 19 members of the public; however, none of them were fatal. Some methods of reconstructive dosimetry have been used to estimate the radiation dose to assist in prescribing medical treatment. The type and development of cutaneous manifestations in the exposed areas of a person is the first achievable gross dose estimation. This review article presents the state-of-the-art reconstructive dosimetry methods enabling estimation of local radiation doses and provides guidelines for medical handling of the exposed individuals. The review also presents the Chilean and Brazilian radiological accident cases to highlight the importance of reconstructive dosimetry.

Biological dosimetry (biodosimetry) is based on the investigation of radiation-induced biological effects (biomarkers), mainly dicentric chromosomes, in order to correlate them with radiation dose. To interpret the dicentric score in terms of absorbed dose, a calibration curve is needed. Each curve should be constructed with respect to basic physical parameters, such as the type of ionizing radiation characterized by low or high linear energy transfer (LET) and dose rate. This study was designed to obtain dose calibration curves by scoring of dicentric chromosomes in peripheral blood lymphocytes irradiated in vitro with a 6 MV electron linear accelerator (Mevatron M, Siemens, USA). Two software programs, CABAS (Chromosomal Aberration Calculation Software) and Dose Estimate, were used to generate the curve. The two software programs are discussed; the results obtained were compared with each other and with other published low LET radiation curves. Both software programs resulted in identical linear and quadratic terms for the curve presented here, which was in good agreement with published curves for similar radiation quality and dose rates.

Aim: The purpose of this paper was to review various dosimeters used in dentistry and the cumulative results of various studies done with various dosimeters. Materials and Methods: Several relevant PubMed indexed articles from 1999 to 2013 were electronically searched by typing “dosimeters”, “dosimeters in dentistry”, “properties of dosimeters”, “thermoluminescent and optically stimulated dosimeters”, “recent advancements in dosimetry in dentistry.” The searches were limited to articles in English to prepare a concise review on dental dosimetry. Titles and abstracts were screened, and articles that fulfilled the criteria of use of dosimeters in dental applications were selected for a full‑text reading. Article was divided into four groups: (1) Biological effects of radiation, (2) properties of dosimeters, (3) types of dosimeters and (4) results of various studies using different dosimeters. Conclusion: The present review on dosimetry based on various studies done with dosimeters revealed that, with the advent of radiographic technique the effective dose delivered is low. Therefore, selection of radiological technique plays an important role in dental dose delivery.

Dimercaptosuccinic acid [DMSA] has been evaluated and used with technetium 99m [[99m]Tc] in imaging of kidneys. DMSA lyophilized kits were prepared and radiolabelled with [99m]Tc. Paper and thin-layer chromatography have been employed using various eluent systems for the radiochemical analysis, percentage labeling and binding capacity of [99m]Tc-DMSA. Female albino rabbits were used for this study. Biological data obtained after intravenous injection of radiolabelled DMSA to female albino rabbits revealed 32.42% uptake and long retention time in the kidneys. On the basis of animal biodistribution data, it is suggested that DMSA when labeled with [99m]Tc is useful complex for renal imaging and can be successfully applied as a diagnostic tool in nuclear medicine. Clinical biodistribution and radiation dosimetry studies are planned in future

Se ha medido la dosis en la superficie de entrada de ojos, tiroides y gónadas en pacientes sometidos a estudio de radiografía de tele de tórax mediante dosimetría termoluminiscente. Se usaron 5 dosímetros en cada una de las exploraciones; encontrándose los siguientes rangos de dosis: 20 mGy a 350 mGy. En el caso de los pacientes pediátricos se propone incluir la información de las dosis recibidas en su cartilla de vacunación con el fin de hacer un seguimiento de los posibles efectos nocivos en su salud.


Doses have been measured at the entrance surface of eyes, thyroid and gonads in patients undergoing chest x-ray studies, using thermo-luminiscent dosimetry. Five dosimeters were used in each investigation, and doses ranged from 20 mGy to 350 mGy. In the case of pediatric patients, it is proposed that dose information from diagnostic examinations is included in the vaccination chart in order to correlate with findings at follow-up.

Introdução: A análise morfométrica dos parâmetros nucleares, tamanho, forma e aspectos da cromatina, mostra-se útil para diagnosticar tumores malignos. O aspirado citológico coletado por agulha fina produz esfregaços suficientemente delgados para a determinação de dimensões nucleares facilmente. Objetivo: Comparar os aspectos morfométricos dos núcleos das células do aspirado proveniente de nódulos de mama. Método: Esfregaços do aspirado provenientes de 71 biópsias de nódulos de mama e determinados como benignos ou malignos, coletados por agulha fina, foram submetidos à coloração de Feulgen e as suas imagens digitais analisadas utilizando-se a Imagilab® 2,3 (Softium Informatica Ltda.), ambientado no sistema operacional WindowsTM 98 SE. Resultados: Dentre os nódulos estudados, 20 foram diagnosticados como benignos e 51 como malignos (8 GI, 23 GII e 20 GIII de acordo com o Sistema de Classificação de Guilford e Robinson). A análise morfométrica da coloração de Feulgen determinou um valor médio para os nódulos benignos de 101,6 e valor médio de 114,3 para os nódulos malignos, respectivamente. A análise morfométrica da coloração de Papanicolaou determinou um valor médio de 103,5 para GI, 131,7 para GII e 98,4 para GIII, respectivamente. Houve uma diferença estatisticamente significante entre o valor médio morfométrico de nódulos benignos e malignos e valor médio entre GI e GII, GII e GIII, mas não entre GI e GIII. Conclusões: A análise morfométrica nuclear dos aspirados de nódulo de mama é um método simples, confiável e complementar, que permitiu distinguir as lesões benignas das lesões malignas da mama.


Introduction: The morphometrical analysis of nuclear parameters, such as size, shape and chromatin features, has shown to be useful for diagnosing malignant tumors. Cytological aspirate collected with fine needle, in turn, provides smears thin enough for easily determining nuclear dimensions. Objective: Compare morphometrical features of cell nuclei in aspirate from breast nodules. Method: Fine needle aspiration smears from 71 breast nodules were analyzed and determined as benign or malignant. They were then Feulgen-stained and had their digital images analyzed using the Imagilab® software package (Softium Informatica Ltda.) run on Windows? 98SE. Results: Among the nodules studied, 20 were diagnosed as benign and 51 as malignant (8 GI, 23 GII and 20 GIII according to Guilford and Robinson Classification System). The morphometrical analysis of the Feulgen-stained material digital images determined an average value for benign nodules of 101.6 and an average value of 114.3 for malignant nodules. The Papanicolaou stain morphometrical analysis determined an average value of 103.5 for GI, 131.7 for GII and 98.4 for GIII. There was a statistically significant difference between the morphometrical average values for benign and malignant nodules, and between GI and GII, and GII and GIII, but not GI and GIII average values. Conclusion: The nuclear morphometry of breast nodule aspirate is a simple, reliable and complementary method that allowed distinguishing benign from malignant injuries of the breast.

OBJETIVO: Para comparar o benefício da mamografia e o risco de câncer induzido por raios X, devese investigar as doses absorvidas. Nesse sentido, determinaram-se espectros dos raios X de um mamógrafo clínico, para combinação alvo/filtro Mo/Mo, utilizando espectrometria Compton, e avaliou-se a dose glandular média (DGM) em um simulador de mamas de BR-12. MATERIAL E MÉTODO:Um detector de CdTe foi usado para espectrometria dos raios X espalhados a ~ 100° por um cilindrode PMMA, para diferentes profundidades de BR-12 e tensões entre 28 e 35 kV. Após a reconstrução do espectro dos feixes primários, a partir dos medidos, determinou-se a DGM. RESULTADOS:Obtiveram-se camadas semirredutoras de 0,39 a 0,45 mmAl (espectrometricamente) e de 0,38 a0,42 mmAl (com câmara de ionização) para os feixes incidentes na superfície do simulador. A DGMNnormalizada por unidade de kerma no ar incidente, na superfície de BR-12, variou de 0,156 a 0,226.CONCLUSÃO: Os valores de DGMN variaram de 1% a 3%, em relação aos obtidos com câmara. O método empregado é uma boa alternativa para a determinação de DGMN e da distribuição de dose em profundidade em simuladores mamários.


OBJECTIVE: To compare mammography benefit and X-ray induced cancer risk, one should investigate absorbed doses. For this purpose, spectra of primary X-ray beams from a clinical mammographyequipment were determined for Mo/Mo target/filter combination,using Compton spectrometry and average glandular dose (AGD) in a BR-12 breast phantom was evaluated. MATERIAL AND METHOD: A CdTe detector was used for spectrometry of X-ray beams Compton scattered around 100°, by a PMMA cylinder, for different depths inside the BR-12 phantom and voltages between 28 and 35 kV. The reconstruction of the primary beam spectra from the measured ones was followed by the determination of AGD. RESULTS: Half-value layer values determined by spectra resulted 0.39 to 0.45 mmAl, and by ionization chamber, 0.38 to 0.42 mmAl, respectively, for beams incident on the phantom surface. The AGDN normalized per unitary incident air kerma, on the BR-12 surface, ranged from 0.156 to 0.226. CONCLUSION: The percentage deviation of AGDN, relative to the chamber measurements, ranged from 1% to 3%. The utilized method is a good alternative to determineAGDN and depth-dose distributions in breast phantoms.

RESUMO

Premature Chromosome Condensation [PCC] appears to have a possible utility for biological dosimetry purposes. The PCC technique may be adapted for cases of suspicion of overexposure where sampling is performed at least one day after an accident. For this purpose, human blood samples were exposed in vitro to [60] Co up to 10 Gy and the PCC technique was performed immediately after irradiation. Analysis of excess PCC fragments distribution showed an over dispersion and the dose-effect relationship was best characterized by linear regression

A utilização da braquiterapia com equipamentos de pós-carga remota com fonte de 192Ir de alta taxa de dose tem aumentado em todo o mundo e no Brasil que conta hoje com 60 equipamentos instalados em Serviços de Radioterapia. A necessidade de se garantir a rastreabilidade da grandeza dosimétrica associada às fontes radioativas à Rede Internacional de Metrologia das Radiações Ionizantes tornou-se evidente e urgente, pois doses altas são aplicadas em pouco tempo ao tumor e qualquer erro na determinação dessa dose pode acarretar muito dano aos pacientes. Na ausência de um padrão dosimétrico estabelecido para o espectro de energia do 192Ir, muitos trabalhos mostraram alternativas para a calibração de fontes de 192Ir de alta taxa de dose com câmaras de ionização tipo dedal e tipo poço. A câmara poço, por ser de uso mais simples e apresentar metodologia reprodutível no ambiente de um Serviço de Radioterapia, passou a ser recomendada por instituições internacionais. Os Serviços de Radioterapia do Brasil que possuem equipamentos de pós-carga remota com fonte de 192Ir de alta taxa de dose utilizam, atualmente, a câmara tipo poço para a dosimetria de suas fontes. Como a calibração desse instrumento ainda é restrita a alguns países do hemisfério norte e a fim de suprir a necessidade do Brasil, este trabalho propõe uma metodologia para a calibração das câmaras poço dos usuários com fonte de 192Ir de alta taxa de dose, rastreada à Rede Internacional de Metrologia das Radiações Ionizantes. No método proposto é utilizada uma câmara dedal, calibrada no LNMRI/IRD/CNEN (rastreado à Rede Internacional de Metrologia das Radiações Ionizantes) em feixe de raios X e 60Co com uma interpolação adequada para a energia do 192Ir. Utiliza-se um sistema de posicionamento dessa câmara e da fonte de 192Ir de um equipamento de pós-carga remota para determinar a grandeza taxa de kerma no ar de referência dessa fonte que é, então, utilizada para calibrar as câmaras tipo poço dos usuários...


The use of brachytherapy with after-loading equipment with remote source of high dose rate 192Ir has increased worldwide and in Brazil, which presents 60 facilities with this equipment nowadays. The need to ensure the traceability of the source dosimetric quantity to the International Ionizing Radiation Metrology Network has become evident and urgent, since high doses values are delivered in a short time to the tumor and any error in the determination of the dose can provoke much damage to patients. In the absence of an established dosimetric standard for the energy spectrum of 192Ir, many studies showed alternative calibration of sources of high dose rate 192Ir, with the ionization thimble or well chamber type. The well type chamber is now recommended by international organisms, because of its simplicity to use and to its reproductibility. Nowadays, it is adopted by all radiotherapy services in Brazil using after-loading equipment with high dose rate 192Ir source. As the calibration of this instrument is still limited to some countries in the northerm hemisphere, in order to enhance the growth of the use of this instrument in Brazil, this paper proposes a methodology for calibration of well chambers of users of high dose rate 192Ir sources, traceable to the International Ionizing Radiation Metrology Network. In the proposed method a thimble chamber is used, calibrated in LNMRI/IRD/CNEN (traceable to the International Ionizing Radiation Metrology Network) in X-ray and 60Co beams with an interpolation for the 192Ir energy. Using a positioning system to hold the chamber and a high dose rate 192Ir source, the quantity reference air kerma rate is determined. This radioactive source is then used to calibrate the user's chambers. The proposed methodology was tested in various radiotherapy services of the country and was also compared to the existing methodology in University of Wisconsin Accredited Dosimetry Calibration Laboratory (UWADCL)...

PURPOSE: To derive and validate an index to correlate the bladder dose with the catheter balloon dose using limited computed tomography (CT) slices. MATERIALS AND METHODS: Applicator geometry reconstructed from orthogonal radiographs were back-projected on CT images of the same patients for anatomy-based dosimetric evaluation. The correlation indices derived using power function of the catheter balloon dose and the bladder volume dose were validated in 31 patients with cervical cancer. RESULTS: There was significant correlation between International Commission on Radiation Units (ICRU)-38 balloon reference dose (Dr) and the dose received by 25% bladder volume (D(25)) (P< 0.0001). Significant correlation was also found between the reference dose of mid-balloon point (D(rm)) and the dose to D(25) (P < 0.0001). Average percentage difference [100 x (observed index - expected index)/ expected index] of observed value of I'25 (index for the dose to D25 bladder with respect to mid-balloon reference point) from that of expected value was 0.52%, when the index was modeled with reference dose alone. Similarly the average percentage difference for I'10cc (index for the dose to 10 cc volume of bladder with respect to mid balloon point) was 0.84%. When this index was modeled with absolute bladder volume and reference dose, standard deviation of the percentage difference between observed and expected index for D(rm) reduced by approximately 2% when compared to D(r). CONCLUSION: For clinical applications, correlation index modeled with reference dose and volume predicts dose to absolute volume of bladder. Correlation index modeled with reference dose gives a good estimate of dose to relative bladder volume. From our study, we found D(rm) to be a better indicator of bladder dose than D(r).

AIM: To evolve a fast dose verification method for high-dose-rate (HDR) brachytherapy treatment plans and to demonstrate its applicability in different clinical cases. MATERIALS AND METHODS: We developed a software tool in VC++ for the Varisource HDR unit for HDR dosimetry plan verification using TG-43 parameters. HDR treatment dosimetry of a number clinical cases using Varisource was verified by comparison with the treatment planning system (TPS). RESULTS: A number of different types of clinical cases treated by Varisource were evaluated. TPS calculated dose values and verification code calculated dose values were found to agree to within 3% for most of the dose calculation points. CONCLUSIONS: We have validated with clinical cases a fast and independent dose verification method of the dosimetry at selected points for HDR brachytherapy treatments plan using TG-43 parameters. This can be used for the verification of the TPS calculated dose at various points. The code is written to work with Varisource, but it can conceivably be modified for other sources also by using the fitted constant of the respective source.
